Requesting Medical Records
Medical records may be released with a completed Release of Information (ROI) / Patient Authorization form, as required by law.
We make every effort to process all Requests for Information (ROI) in a timely manner.
Processing Timeframes

Emergency care: Immediate

Continuation of care (requests from health care facilities): Within 24 hours

All other requests: Within 10 business days
Fees
Fees may apply to requests that are not sent to another health care facility for continuation of care.

Fees are collected prior to the release of medical records
