Whether you’re recovering from surgery, managing a chronic condition, or working to stay independent in your daily life, Three Rivers Hospital’s rehabilitation team is here to help.

We offer one-on-one, personalized care in an outpatient setting, and we’re available to patients during a hospital stay as well. Our therapists work closely with your physician and care team to make sure your treatment is coordinated and moving in the right direction.

Physical Therapy

Physical therapy helps you move better, hurt less, and get back to doing the things that matter to you — whether that’s returning to work, staying active, or recovering after surgery.

We treat conditions including:

  • Joint pain — shoulder, back, knee, neck, hip, and ankle
  • Post-surgical rehabilitation, including total hip and knee replacements
  • Work-related injuries (Labor & Industries)
  • General orthopedic and musculoskeletal conditions

What to expect

Your physical therapist will take a hands-on, whole-person approach to your care. Appointments are longer and more individualized than what you might find at a larger clinic — so you get focused attention on your specific goals, not a one-size-fits-all plan.

The work you do between appointments matters too. We’ll make sure you leave every visit knowing exactly what to do at home until we see you next. Patients who stay consistent with their home exercises see the best results.

PT appointments are available weekdays. For inpatients recovering at Three Rivers Hospital, our physical therapist can also see you during your stay and help coordinate your care as you head home.


Occupational Therapy

Occupational therapy helps you maintain or regain your ability to participate in the daily activities that define your life — from getting dressed and preparing meals to pursuing hobbies and returning to work. It’s not just for job-related injuries; OT supports anyone who wants to preserve their independence, restore their routines, or adapt after an illness, injury, or health change.

A referral from your provider is required to get started. Ask your doctor or care team, or call us and we can help point you in the right direction.

We work with patients recovering from:

  • Stroke and neurological conditions
  • Spinal cord injuries
  • Orthopedic injuries and post-surgical recovery
  • Work-related injuries
  • Chronic conditions affecting daily function

What to expect

Your occupational therapist, Jordan Smith, takes a client-centered, evidence-based approach — which means your goals drive your treatment plan. Every patient’s definition of independence is different, and your care will reflect that.

Jordan also assists with durable medical equipment (DME) fittings and adjustments to support your safety and function at home.

OT appointments are currently available Monday through Thursday, 9:00 a.m. – 3:00 p.m. Hours are subject to change; call us to confirm current availability.

For patients admitted to Three Rivers Hospital or receiving skilled nursing care, Jordan can also work with you during your stay and help coordinate your care as you transition home.

Please note: We do not provide hand therapy or pediatric occupational therapy services at this location. If you need a more specialized referral, we’ll help coordinate that as part of your care.
