Three Rivers Hospital is a public Critical Access Hospital. In accordance with the Washington State Public Records Act (RCW 42.56), we welcome public records requests. Please review the information below if you wish to submit a request to our Public Records Officer.

Cost for Public Records Copies

Electronic copies: No cost.

In-person inspection: No cost.

Hard copies: 15 cents per page for black-and-white copies, plus postage and shipping costs if applicable.

Helpful Information to Include in a Written Request

  1. Your name and mailing address;
  2. The date of the request;
  3. A detailed description of the public record(s) being requested;
  4. Whether you want copies, or want to inspect the requested records in person at the hospital;
  5. The address where copies are to be sent (if copies are requested); and
  6. A statement regarding whether the records are being requested for a commercial purpose.

You will receive a response from Three Rivers within five (5) business days.
